Where are you located?

Our office is located at 6603 Oak Hill Boulevard, Suite B, Tyler, Texas. We share the building with East Texas Foot and Ankle. Upon entering the building, you'll find us up the ramp on the left side in Suite B.

What is the fee for services?

Our comprehensive assessment fee, which includes the test day, scoring, report writing, and a feedback session, is $2,300. A $500 deposit is required to schedule, and the remaining $1,800 is due on the day of testing.

Is there a deposit for services?

Yes, a $500 deposit is required when scheduling your appointment. This deposit reserves your testing slot and is applied to your total assessment fee. If you need to reschedule, your deposit carries forward to the new date; if you cancel, the deposit is non-refundable.

Do you offer a payment plan?

At this time, we do not offer a payment plan.

Do you take insurance?

We do not bill insurance directly. However, we can file an out-of-network claim on your behalf. Additionally, we will provide a Superbill that you can submit to your insurer for reimbursement. Since coverage varies by policy, our team will consult with your insurance provider to determine your specific out-of-network benefits, ensuring you have accurate information about potential reimbursements before you begin our services.

How long is the testing appointment?

The testing appointment typically lasts around four hours, during which we complete all necessary assessments in a single session.

When will I get the report?

You will receive your report within 7 to 10 days after the completion of testing.

Will my results be explained?

Absolutely! We include a feedback session to discuss your report and answer any questions you may have.

Can testing help me receive school accommodations?

Yes, our testing can assist you in obtaining school accommodations. We provide documentation for IEP/504 plans and other general school accommodations.

Is your dog always in the office?

Our office dog, Ranger, is sometimes present. However, if you have allergies or a fear of dogs, Ranger can be temporarily removed from the office upon your request. Please indicate your preference in your initial paperwork.

Are services confidential?

Yes, all our services are confidential. We adhere to HIPAA regulations, ensuring the protection and confidential handling of your personal health information. Unauthorized access or disclosure of your health records is strictly prohibited.
