Yes, a $300 deposit is required when scheduling your appointment. This deposit secures your testing slot and allows you the flexibility to reschedule as needed, ensuring we hold your appointment for you.
Our payment plan is structured as follows: $833.33 is due at the time of your testing appointment, followed by $683.34 due 30 days after the appointment, and a final $683.33 payment due 60 days post-testing.
We do not bill insurance directly. However, we can file an out-of-network claim on your behalf. Additionally, we will provide a Superbill that you can submit to your insurer for reimbursement. Since coverage varies by policy, our team will consult with your insurance provider to determine your specific out-of-network benefits, ensuring you have accurate information about potential reimbursements before you begin our services.
